Andi Garcia-Ortiz/Dr. Carli: Week 2 (June 12th - 16th, 2023)

Monday (June 12th, 2023)

I attended Dr. Carli's clinic on the 7th floor of the Weill Cornell East River Professional Building. I got to see Dr. Carli consult over 20 patients from 8am-6pm. Dr. Carli always uses models of knee and hip replacements to explain the procedure step by step to his patients. He states the "universal truths" of knee replacements, 1) The replaced knee is NOT as a youthful one, 2) one in five patients are unhappy with their knee replacement, 3) the recovery process is painful. Prior to surgery, the patients should try minimally invasive treatments such as physical therapy and cortisone injections. Dr. Carli always examines his knee replacement-seeking patients, gauging the sensation in the bottom and top of the feet, determining baseline movement in non-impacted knee, and finally determining any leg length discrepancies. Dr. Carli performs 100-150 hip replacements a year. 

Tuesday (June 13th, 2023)

The Prosthetic Joint Infection (PJI) research groups have their weekly meetings at 8am at HSS Research Institute on the 6th floor. The group is led by Dr. Mathias Bostrom and Dr. Alberto Carli. The meeting discussed the accepted Orthopedic Research Society (ORS) conference abstracts. I also submitted my health form for approval to work with the mice in the vivarium. I also made agar plates from scratch to culture E. coli for our upcoming experiments.

Wednesday (June 14th, 2023)

I shadowed Dr. Carli and his Physician Assistant Lexi Simon on the 4th floor OR in HSS. I shadowed a total hip replacement with the Mako Robot and a medial knee replacement. I left early to meet with Dr. Brett Steineman to discuss his research. Dr. Steineman is a scientist at HSS who is researching techniques to improve clinical care of foot and ankle pathologies; and knee meniscus injuries, using computational models for both. I missed an infected hip revision in the OR so I am going to follow up with Dr. Carli on Monday to discuss the x-rays and final outcome.

Thursday (June 15th, 2023)

I worked at the HSS Research Institute on the 4th floor with Christina Chao. I completed all my online laboratory training.

Friday (June 16th, 2023)

I shadowed Dr. Carli and his Physician Assistant Lexi Simon on the 4th floor OR in HSS. I shadowed three total hip replacements and two total knee replacements. The first two hip replacements involved cement due to the patient's older age and declining bone quality. I also saw the implant room that replaces all the orthopedic implants for the OR floor.

 This summer has gone by so fast! This week was mostly just wrapping up the summer. I went and saw a little bit more of the mouse gait analysis and acclimation. Learning how to acclimate the mice for the gait analysis is super important for the consistency of the data, as mice that haven’t been acclimatized to the treadmill have a much wider spread and learning curve over the experiment. We learned how the mice get tire after about a minute of running, even if you spread that minute out over a couple bouts of running, which was super important for the next couple days of acclimation.  I also got the opportunity this week to shadow in the Gait and Motion Analysis lab for a case. I have never seen the human gait analysis before, so I was immensely grateful for this opportunity! Ellen Brooks/Dr. Robert Marx - Week 1

One action packed week complete already! The first few days of this week were filled with orientation meetings, introductions to Dr. Marx and his clinical team, and tours of the research facilities.  On Monday, I started shadowing Dr. Marx, his resident Dr. Braum, and his PA, Joe, through clinical rotations. As a person that has been in the patients chair many times, it is fascinating to see the doctor-patient interaction from the other side and to start to notice how the Dr. Marx is able to connect abnormalities in flexibility and gait to possible diagnoses.  I continued my shadowing of Dr. Marx late in the week in the Operating Room. Marysol Chu / week 8

  It’s hard to believe that summer immersion has come to an end.     The past two months were extremely valuable in expanding my knowledge of the clinical aspect of my research. I was able to build relationships with clinicians in the field actively working on clinical trials and allowed me to interact with patients and identify needs that still exist despite all the advancements that have been made in medicine.     I could not have asked for a better clinician to shadow throughout the summer. Dr. Gomez made sure I saw as much as I could not just within bone marrow transplant but with other clinicians in the hematology/oncology department. I remember the first couple of weeks were emotionally challenging because of the severity of the complications that some of the patients experienced. It was very difficult for me to see them and their families struggling but at the same time I could see the medical team fighting and doing everything they could for the patient....
